The ultimate goal is to support the systematic adoption, implementation, and evaluation of successful programs, practices, and policy changes. The database provides carefully reviewed, documented, and ranked practices that range from good ideas to evidence-based practices.

Goal: The goal of this program is to improve colorectal cancer screening rates among older adults.
Impact: Participants in the intervention group had significantly higher colorectal cancer screening attendance, as well as having more positive attitudes about screening and placing a higher priority on screening.

Goal: The Communities That Care Coalition's mission is to bring Franklin County schools, parents, youth and the community together to promote protective factors, reduce risk factors, prevent substance use and other risky behaviors, and improve young people's ability to reach their full potential and thrive.

Goal: The purpose of the Interlocal Association's (IA) Youth with Disabilities Project was to support demonstration projects to help Workforce Investment Act (WIA)-assisted youth programs develop the capacity to serve youth with disabilities. Although youth with disabilities had been served under previous employment and training programs during the latter years of the Job Training Partnership Act (JTPA), with the implementation of WIA, services such as work experience, summer employment, and others decreased substantially. IA looked to expand and enhance the quality of services to youth with disabilities.

Addressing the Social Determinants of Health through the Alameda County, California, Place Matters Policy Initiative (Alameda County, CA)

Goal: The goal of the Place Matters initiative is to improve the health of participating communities by “addressing social conditions that lead to poor health” through “identifying the complex root causes of health disparities and defining strategies to address them.”
Impact: AC Place Matters has identified 10 best practices that could help other programs address social determinants of health: find and foster strong leadership, dedicate staff resources to the work, engage staff from across the local health department, contribute to building grassroots power, address root causes, partner with community organizations and leaders, partner with government institutions across sectors, work reactively and proactively, build capacity, and use tools that ensure a focus on health equity.

Goal: The mission of MOMS Orange County is to help mothers and their families have healthy babies by providing health coordination, education, and access to community services. MOMS Orange County’s vision is that all babies born in Orange County are healthy at birth.
Impact: Measures such as the percent of babies born at a low birth weight, percent of babies born premature, and the percent of babies admitted to the NICU were all markedly better for program participants when compared to many comparison benchmarks.

Impact: The Community Preventive Services Task Force (CPSTF) recommends depression care management in primary care clinics for older adults with major depression or chronic low levels of depression (dysthymia) on the basis of sufficient evidence of effectiveness in improving short-term depression outcomes.

Goal: The goal of the program is to increase fruit and vegetable consumption behavior in participants of the Women, Infants, and Children program in Genessee County, Michigan.
Impact: Participants of the program increased their fruit and vegetable consumption and the program had a positive effect on participants attitudes toward consuming fruits and vegetables.
