Goal: The mission of HKES&I is to increase access to needed healthcare for children. This is accomplished through the goal of identifying children who need further follow-up, particularly for hearing and vision problems, and to connect them to those services.

Impact: Healthy Kids Express Screening and Immunization program is successful in providing evidence based care to low-income populations and for connecting those identified with screening needs to needed follow-up care.

Goal: The program aims to accomplish two things in Central California: (1) permanently change local policy with respect to existing operating procedures in school districts and schools to help reduce exposure of students, teachers, staff and nearly communities to outdoor environmental asthma triggers ;and (2) provide education on air quality and potential health effects from exposure to air pollutants.

The mission of Teen Advocates for a Healthy KCK is to empower underserved, at-risk adolescent girls to live healthy lives filled with positive options with the vision that girls, when healthy in mind, body and spirit, will be strong leaders and advocates for thriving, healthy families and communities.

Impact: Teen Advocates for a Healthy KCK helped girls discover their personal power and develop skills and behaviors that led to healthier lifestyle choices.

Goal: The goal of this program is to improve health care accessibility and availability for low income, uninsured and underinsured residents of Marion County.

Goal: The goal of this program was to increase immunization rates in the Butler County area.

Impact: Immunization rates increased from 53.2% to 72.4% and parents reported a better understanding of the importance of timely vaccinations.

Goal: The goal of the Comprehensive Diabetes – Lower Extremities Amputation Prevention initiative is to improve foot care and reduce lower extremity amputations among people with diabetes.

Goal: The mission of the program is to work vigorously to free participants from the burden of welfare dependency, and achieve a better, happier lifestyle through self-sufficiency. It will serve the taxpayers of Riverside County by reducing welfare dependency, thus making tax dollars available for other expenditures and needs.

Impact: The program produced a large net savings to the government through increased tax revenues and reduced welfare and food stamps payments (as an estimate, $2.84 saved for every $1.00 invested over five years).

Goal: The goals of the MCM are to provide a comprehensive and centralized concussion care program to 1) increase concussion awareness and identification through education and training; 2) facilitate the return to play decision with effective medical treatment, which includes baseline neurocognitive testing; and 3) implement a standardized concussion care protocol and concussion injury surveillance system to assist in the prevention of concussions, improve player safety, and limit school liability.

Impact: From the pilot evaluation of the model it was determined that the MCM model or a similarly designed one is effective in increasing the number of concussions identified, reported, and also treated at a clinic.
